Care pathways and referrals

Victorian and Tasmanian PHNs work to ensure patients receive the right care, in the right place, at the right time. To do this, they collaborate with health professionals to develop local care pathways.

Local care pathways

These are online suites of detailed information on testing, diagnosis, treatment and referrals for hundreds of conditions. Referral options are optimised for health professionals in each region.

They are designed as in-consult decision support tools for clinicians.

Clinical working groups regularly review and update pathways in line with any changes to the evidence base, and to align with statewide referral criteria for Victoria and Tasmania.

PHNs independently operate the care pathways programs in their local region.
