North Western Melbourne Primary Health Network (NWMPHN) is seeking to commission an organisation to provide an independent evaluation of the statewide Optimal Care Pathways HealthPathways project.
NWMPHN is the lead PHN for the project and is performing this commissioning activity on behalf of the Victorian and Tasmanian PHN Alliance (VTPHNA).
The Optimal Care Pathways (OCP) project aims to improve cancer outcomes by strengthening primary health systems to deliver appropriate and high-quality cancer care for all Victorians regardless of where they live.
The purpose of this tender is to commission an external evaluator to provide a comprehensive formal process and outcome evaluation of the project. The successful respondent will work in collaboration with the Victorian PHNs and other key project stakeholders to develop and implement the project evaluation.
