    Dementia Support Pathways and Implementation Project

    The Dementia consumer resources and directory have been developed as part of the Dementia Support Pathways and Implementation Project. This activity is supported by funding from the Australian Government, through the PHN program.

    The aim of this initiative is to support people living with dementia to have better access to dementia care and live well in the community, for as long as possible.

    This initiative will support clinicians, primary care and the allied health workforce to enhance the care and support provided to people living with mild cognitive impairment or dementia, as well as their carers and family by:

    • improving the timeliness of dementia diagnosis
    • increasing the uptake of post-diagnostic services and supports.

    As part of this project Victorian and Tasmanian HealthPathways teams will deliver:

    • dementia specific pathways using HealthPathways, for use by clinicians during consultation with patients, to support assessment referral to local services. This will reflect the optimal pathway for seeking dementia diagnosis as well as post-diagnostic supports including local services/supports that are available in the region.
    • consumer-focused dementia support resources which detail the post-diagnostic care and support available for people living with dementia, and their carers and families, within their local area.

    Communications resources

    To assist PHNs in promoting the new dementia supports and resources to consumers and health professionals two toolkits have been created.

    The toolkits include the copy and graphics, as Indesign files which will need to be customised by each PHN with logos (and dates for the Demystifying Dementia training tiles/banners).
