Carer Awareness in General Practice: Information for PHNs

    Home Carer Awareness in General Practice: Information for PHNs

    This project has been funded by DHHS through Carers Victoria and delivered by the VTPHNA, led by North Western Melbourne PHN.

    Our agreement with DHHS is to develop and promote the Carer Awareness in General Practice education to help improve the lives of the many carers in our community.

    Each PHN is required to deliver two education activities within their contract period. Each PHN will promote and deliver the education as they wish, as face to face or live webinar sessions supported by the slides with a local presenter, or as a webinar pack. All the tools you need to promote, run and evaluate the sessions, as well as the process for GPs to claim professional development points are provided below.

    Evaluation

    For a face-to-face education session or local webinar:

    An evaluation survey should be used to evaluate your sessions You are welcome to add questions to the evaluation survey template (.docx), but you must not edit or remove any of the existing questions.

    Evaluation results can be reported to NWMPHN using the reporting template above.

    For the recorded webinar and online education:

    A survey link is included on the education webpage. Collation and reporting of these survey results will be managed by NWMPHN.

    • PHN education session slide deck (available as a PDF or PowerPoint deck)

    Instructions for CPD points

    The education session and webinar content has been approved by RACGP for two CPD activity points and ACRRM for one hour of professional development (2020-2022 triennium). NWMPHN will manage these points on behalf of the other Victorian PHNs.

    For a face-to face session or local webinar

    Complete the professional development points pro forma form (.docx) at the end of your session and return it to education@nwmphn.org.au

    NWMPHN will submit these to RACGP/ACRRM for eligible attendees to obtain their points.

    For the recorded webinar and online education

    A participant will be prompted to complete and submit a survey after watching the three-part webinar/online education. If the survey is completed successfully, NWMPHN will submit the participant details to RACGP/ACRRM.

    Please promote that participants must successfully complete the survey at the end of the webinar to receive professional development points.
